Summary of Position

Lompoc ACT is designed to engage with and provide services to adults with serious and persistent mental illness in the community where they live, instead of a hospital or clinical setting. Rather than brokering services, the ACT team provides treatment, support and rehabilitation services directly with a "whatever it takes" approach. The program provides a full range of services including assessment, individualized treatment planning, personal service coordination, integrated co-occurring drug and alcohol services, medication support, counseling, social skills development, supported housing, integrated vocational services, supported education and access on a 24/7, 365 day basis. 

The Team Leader will work both as a manager and team member for a 17-person ACT team, with a staff-to-consumer ratio of approximately 1 to 10. 

More specifically, the Team Leader will be responsible for crisis care, case management service delivery, individual rehabilitation activities, skills building, transportation and all aspects required to help clients succeed. The Team Leader will establish, administer, manage and lead the Lompoc ACT program; will train, supervise and evaluate the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program clients; and will function as a practicing clinician on the team.

RESPONSIBILITIES and DUTIES

1.    Provide a recovery-oriented, wellness centered approach to providing services.

2.    Services will be delivered with a "whatever it takes" approach, collaborating with the team to be creative in delivering services and meeting consumers "where they are" in their recovery.

3.    Seek trainings in proven systems of service delivery such as Assertive Community Treatment, motivational interviewing, co-occurring disorders, Dialectic Behavior Therapy, CBT, and ongoing discussion and education with team members that promotes a learning environment.

4.    Work effectively with team members that have self-disclosed as consumers of mental health services and/or are recovering from drug or alcohol abuse.

5.    Direct the day-to-day clinical operations of the team including scheduling staff work hours to assure 24/7 coverage; daily team meetings and assessment planning meetings; continuously evaluate the status of clients; coordinates and plans treatment activities to ensure attention to their needs.

6.    Direct and coordinate the client engagement and admission process and the treatment, rehabilitation, and support services of the program in coordination with the psychiatrist. Schedule the admission interview; review the initial assessment and initial treatment plan; assign appropriate staff to the individual treatment team (ITT); and provide clinical supervision for the development of the comprehensive assessment and the treatment plan for each client. Conduct clinical assessments and provide direct clinical treatment, rehabilitation, and support services to clients.

 7.    Direct and coordinate, for each client, the comprehensive assessment of psychiatric history (e.g. onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g. self-care, living situation, nutrition, money management); and family structure and relationships.

 8.    Participate in staff recruitment, interviewing, hiring, work assignments, evaluation, review, orientation and performance supervision according to work rules, regulations, and policies; schedule and implement staff orientation and training; and manage student/intern and volunteer training.

 9.    Motivate a team with individual competencies, such as co-occurring disorder specialist, consumer background, and employment specialties, to enhance the team's ability to connect with and deliver recovery services to all clients.

 10. Maintain appropriate ACT office space including office equipment, computer, and telephone lines. Follow and report on HIPAA regulations/violations, complete and follow-up on ADMHS incident reports, maintain MediCal site certification.

11. Administer the ACT program budget including the purchase of necessary furniture, equipment, communications devices (e.g. telephones, cellular phones), and office supplies. Assure that employees are in compliance with transportation (e.g., insurance and CDL for use of vehicles) requirements.

12. Supervise medical records management assuring maintenance of the medical record in compliance with ADMHS policies, Medi-Cal, and other third-party requirements; train staff on medical record requirements; regularly review client assessments, treatment plans, and progress notes written by the staff; and supervise individual staff for billing and client records mastery.

13. Carry out and document quality-assurance activities and review use of program services.

14. Develop and update program policies and procedures and revise as necessary. 

15. Initiate and maintain relationships, in coordination with other staff, with law- enforcement, other service agencies, and with informal community resources (e.g., landlords, employers).

16. Perform 24/7 on-call coverage for crisis intervention and for staff supervision and consultation.

17. Carry a caseload.

18. Position may be required to transport consumers in one's privately owned vehicle.

19. All other duties as assigned.

Environment

1.    Moderate, physical activity and frequent driving; may include moving or lifting up to 25 lbs.

2.    Position supervises all staff in the program and supports the team approach to meeting program objectives.

3.    Involves moderate risks that require safety precautions and the risk of exposure to potentially dangerous situations.

MINIMUM REQUIREMENTS

1.    Licensed MFT, LCSW, LPCC or an intern working towards licensure; waivered PsyD or Psychologist Doctoral candidate with at least 1 year of direct experience in management and at least 2 years of experience with adults with severe persistent mental illness.

2.    At least 3 yrs experience with persons with severe and persistent mental illness and prefer 50% of intern hours completed.

3.    Must have. Strong interpersonal skills and oral/written communication skills.

4.    Ability to handle difficult situations, display good judgment and decision making skills, and remain calm and clearheaded under pressure.

5.    A high level of organizational and time management skills. Must be skilled at using multiple computer programs such as Outlook and Word.

6.    Must use reasonable precautions in the performance of one's duties and adhere to all applicable safety rules and practices while acting in such a manner as to assure at all times, maximum safety to one's self, fellow employees, clients, and other persons contacted through TMHA business.

7.    Conditions of employment include:

·      Must be at least 18 years of age; possess a valid CA Driver's License, a minimum of 2 years driving experience, reliable transportation, proof of auto insurance, and DMV driving record that meets TMHA's current vehicle insurance requirements. May be required to utilize personal vehicle and auto insurance.

·      Attendance is an essential function of this job that requires face-to-face interaction with clients and/or other employees.

·      Ability to possess and maintain good physical and mental health.

·      Comply with a T.B. skin test or chest x-ray indicating a negative tuberculosis result and receipt of a criminal record clearance through fingerprinting.

·      Must be able to successfully complete First Aid and CPR training.
